CANCELLATION POLICY:

Please give 24 hours notice before cancelling an appointment. Less than 24 hours notice can result in missed visit fees. 

CHECK-IN:

When checking in for your first visit, please arrive at least 15 minutes prior to your scheduled appointment time to complete new patient paperwork. Return visits should arrive 10 minutes before their scheduled visit time to check in and verify insurance and address information. 

Prescription refills:

All prescription refills need to be initiated directly through your pharmacy. Please plan 2-3 business days for your prescriptions to become available once you have contacted your pharmacy.

Email communications:

Email is not to be used in the case of an emergency. Established patient emails regarding a new complaint, involving a new treatment, or involving treatment of an acute complaint will incur a fee. Established patient emails regarding a reaction to a prescribed treatment, clarification of a prescribed treatment, or when you have been asked to communicate via email, a fee will NOT incur. If there is a medical emergency, please go to the nearest ER or call 911.
